Girl, 14, died in a hospital with 'chaotic' A&E staff who inserted medical equipment upside down when they tried to save her
Jessica Ashton-Pyatt suffered two heart attacks after she vomited so violently her stomach ruptured in October last year
She was rushed to Pilgrim Hospital in nearby Boston, but died hours later
Inquest heard A&E staff were ‘overwhelmed’ when Jessica was admitted
This led to a catalogue of errors including a situation where a defibrillator could not be used because it was left unplugged by staff
Verdict of death by natural causes was recorded by the coroner, however

One in four A&E patients are there because they couldn't get a GP appointment: Nearly six million visits are simply because a family doctor was not available
5.8million visits directly down to patients being unable to see doctor
Last week, number of patients at casualty reached a record high
Patients finding it increasingly difficult to get a GP appointment

Is this GP showing how to solve the A&E crisis? Steffi's working in Casualty to weed out patients who don't need hospital care - with remarkable results
Last year, a record 24  million people went to Casualty
One in four said they were there as they couldn't get a GP appointment
At Musgrove Park Hospital in Taunton, Somerset, they have a new strategy
Seven days a week there's a GP in A&E, such as Dr Steffi Bridgman
