Girl, 10, dies after series of hospital blunders including expired drugs, lack of senior doctors and an on-call locum who decided to stay at HOME when contacted

Arina Durbazeva was taken to Basildon Hospital after having a fit
At the hospital she started fitting again and doctors were unable to give her the drugs she needed because they were past their use by date
No senior doctors were on the ward and the on-call consultant decided to stay at home when Arina's condition deteriorated and he was called
Arina had a heart attack and stopped breathing for 14 minutes
Was moved to St Mary's Hospital in London but died two days later
The coroner recorded a narrative verdict

He was admitted after suffering chronic diseases and a collapsed left lung
Inquest heard doctors took him to theatre for a procedure but in a fatal error the surgeon operated on the right lung

Researchers calculated 750 deaths prevented in a year at the hospitals
Nurses are meant to measure patients’ heart rate, blood pressure and oxygen levels
Often these readings aren’t taken regularly enough, sometimes an 8hr gap
To address the problem, hand-held devices were given to nurses at Queen Alexandra hospital in Portsmouth in 2005 and University Hospitals in Coventry in 2007
They were handed either iPhones or iPads installed with an app that takes patients readings and reminds nurses when they are next needed
