The Department of Health are seeking views on long term conditions,
http://www.dh.gov.uk/health/2012/05/ltcstrategy/

The DoH states that:

"We would like to hear from people with long term conditions, carers, health professionals, commissioners, local authorities and the voluntary sector. People’s experiences, both positive and negative, and their ideas about what could be done differently, are vital to make the strategy as relevant as possible.

"The content of the strategy will not be confined to health issues. It will also cover other aspects of people’s lives that can be affected by long term conditions and bring together departments across central government to sign up to shared aims."

The new Long Term Conditions site is at http://www.dh.gov.uk/health/category/policy-areas/nhs/long-term-conditions/

Re: Long Term Health Condition (24th Jan 13 at 11:02am UTC)
Norman Lamb MP has answered a Parliamentary Question about care plans for people with long-term conditions.

Linda Riordan MP asked the Secretary of State for Health what steps his Department plans to take to improve awareness of the commitment in the NHS mandate that everyone with a long-term condition will be offered a care plan.

Norman Lamb said that the mandate sets out the Government's ambitions for the health service for the next two years, which includes an objective around offering care plans for those with long-term conditions. The NHS Commissioning Board is legally required to seek to achieve the objectives in this document.

Furthermore, he stated that it is for the Board to decide how it will deliver its objectives, including raising awareness, and that the Government will hold the Board to account for progress.
